Success Stories
The mission of the Virginia Diabetes Council to "bring people together to identify and promote best practices for diabetes prevention, control and treatment in Virginia." One strategy toward achieving this mission is the Virginia Diabetes Council Success Story Project which is compiling stories illustrating successes and best practices in diabetes prevention, control and treatment from partners across the state.

Submitting a Success Story:
To submit a success story to the Virginia Diabetes Council, click here to download a one page form with the requested information and send to our Administrative Assistant at ed@virginiadiabetes.org. Submissions will be reviewed monthly and any recommended changes will be returned to you before posting on the VDC Web site. Each story should be one page in length, so please be concise with your information.
Your Stories Will:
- Be a program example for other partners and organizations - The VDC is committed to identifying and promoting innovative models for Diabetes related efforts and interventions.
- Educate policy makers about programs that work - State specific programs make a difference in the fight against diabetes and other chronic diseases.
- Show decision-makers that resources are well-invested - Legislators and other decision-makers want to see how funds are being used to target key populations with evidence-based interventions that prevent and control diseases and risk factors. More than 75% of the nation's medical care costs are spent caring for people with chronic conditions could be postponed or prevented, saving medical care dollars.
